What you should know about Flemingdon Health Centreopen
Flemingdon Health Centre (FHC) is a registered charity and an incorporated not-for-profit Community Health Centre (CHC). Together with those we serve, food security, employment, such as poverty, we address identified issues, isolation and newcomer support. Our programs and services emphasize community involvement, health promotion and disease prevention. Along with the provision of family medicine, FHC also focuses on the factors that affect our community's health. We are primarily funded through the Ministry of Health/Toronto Central Local Health Integration Network (TC-LHIN). We provide a range of health related services based on the social determinants of health and community engagement models. We also work in close partnership with other agencies and organizations to ensure efficient service delivery.
